Camper Health Form. Resident campers need the form filled out with the doctor's physical dated within 24 months of their time at camp. Day campers just need the health history filled out by parent/guardian.
To apply for financial assistance, fill out this application and turn it in to camp and we will be in contact with you. It is our belief that no child shall be turned away due to an inability to pay.
